Please call for the rates as the rates differ based on the type of testing.
Insurances
Dr. Cook does not accept insurance. However, she will provide a superbill for you to submit to your insurance company. Your insurance company may or may not reimburse you.
Payment
Cash, check, and all major credit cards are accepted for payment.
Cancellation Policy
If you do not show up for your scheduled therapy appointment, and you have not notified us at least 24 hours in advance, you will be required to pay the full cost of the session.
